Your First Appointment

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:

• Patient's insurance card
• List of current prescriptions and/or over-the-counter medication, including dosage and frequency
• Information about patient's medical and surgical history

Medication Refills

To refill your prescription, please contact your pharmacy. Your pharmacist will contact our office. Have your pharmacy send an electronic request, not a fax. Please allow 24 hours to process your refill request.

Insurance and Payment Information

Insurance Providers

It is the patient's responsibility to notify us of an insurance change and to notify us if you have a deductible or what your co-pay/co-insurance is.

Bay View Psychiatry accepts many insurance providers. Below is a brief list of the currently accepted insurance providers.

• United Healthcare

If your insurance company is not listed, please contact us and we will determine your eligibility.

Payment Information
We accept cash, checks, and all major credit cards. Make checks payable to Marc Reiner, MD.

Privacy Practices

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients, and a copy is available upon request.
